Clients: Emotional Support Animal Letter

Clinicians may provide emotional support animal letters for their clients at their discretion. Emotional support animal letter may assist clients with obtaining an appropriate accomodation for housing, travel, or work. Progress Counseling Group, its affiliates, and clinicians do not represent, warrant, or guarantee that a requested accomodation will be granted.

Requirements

  1. Client must acknowledge consent via electronic signature in Theraplatform prior to assessment and determination.
  2. Clinician must complete and provide letter using the ESA Letter form in the provider portal.
  3. Registered Interns must receive approval from Clinical supervisor prior to advising clients that a letter may be provided.

Fees

The published price for ESA Letters is $125.00. Clinicians may adjust price at their discretion.


Sending Theraplatform consent form

  1. Access client account
  2. Click on CONSENT FORMS tab
  3. Select “Emotional Support Animal Disclosure” from drop down then click Send Now button.

Creating Letter in Provider Portal

Access form Provider Portal drop down.

Complete all required fields – Click Create Letter button

Client receives letter via email, clinician receives copy for records.
