HIPAA Release of Information

A HIPAA release of information is a legal document that allows a patient to authorize healthcare providers to share their protected health information (PHI) with specified individuals or organizations. Under the Health Insurance Portability and Accountability Act (HIPAA), patients have the right to control who can access their medical records and how that information is used.

The release typically includes:

  1. Patient Information: Details about the patient, such as name and date of birth.
  2. Recipient Information: The person or organization authorized to receive the information.
  3. Scope of Information: A description of what specific information can be shared (e.g., medical records, billing information).
  4. Purpose of Disclosure: The reason for sharing the information (e.g., for treatment, legal purposes).
  5. Expiration Date: When the authorization expires or if it is ongoing until revoked.

Clients must complete a separate form for each person or organization they choose to release information to. Patients can revoke this authorization at any time, which will stop future disclosures, but it won’t affect information that has already been shared based on prior consent.

The HIPAA Release of Information has been integrated into the Theraplatform Consent Form process. Providers can choose to send the consent form at any time.
